Small Business Subcontracting Forms

These forms are required for the completion of the federally required documentation needed for the Small Business Subcontracting Plan.  See the Federal Requirements webpage for more information.

  1. Small Business Subcontracting Plan Worksheet - Must be completed by Principal Investigator and Contract Administrator so that Supplier Diversity Program Coordinator can complete the Small Business Subcontracting Plan documentation listed below.
  2. Master Cover Letter – template used by Supplier Diversity Program Coordinator to enter information relevant for plan being submitted.  Both the Supplier Diversity Program Coordinator and the Principal Investigator must sign this letter before it can be sent by the Sponsored Projects Office to the federal agency.
  3. Master Subcontracting Plan Notice – Standard notice to federal agency used by Supplier Diversity Program Coordinator to enter information relevant for plan being submitted. 
  4. Addenda A & B – Excel worksheets which provide supporting documentation for specific percentage and dollars proposed for purchase of materials and supplies, equipment over $5,000, and travel from small business suppliers.
For Ongoing Use during the Course of a Contract
Small Business Utilization Worksheet – Federal Acquisition Regulations and the documentation that the PI signs when submitting the plan and the final contract require that the PI and the research team track and be able to provide an explanation when they are unable to use small businesses as they have projected on the approved Small Business Plan. This form is a suggested worksheet for documenting when small businesses cannot be used.
